The Transformation Readiness Blueprint

30 questions. Six dimensions. About four minutes. Score how ready your organization actually is for a business transformation initiative — before you commit budget, a hire, or your credibility to it.

Check every statement you can honestly claim.

Be ruthless — the boxes you can’t check are the ones that end tenures and burn budgets. Your score updates live as you go, and the scale below borrows its language from Six Sigma belt levels, on purpose: readiness is a discipline you build, not a title you’re handed.

What this Blueprint does — and doesn’t — tell you.

This self-assessment scores readiness across six dimensions — mandate, culture, process and data maturity, organizational structure, politics, and runway — using thirty statements drawn from the patterns that separate transformation efforts that stick from the ones that quietly die in year two. It’s designed to surface where the real gaps sit, not to replace a diagnosis.

What it doesn’t capture: the specifics of your processes, your data, or your politics. A formal DMAIC Measure phase does that — baselining the metrics that matter and mapping the stakeholders by name. This Blueprint starts the conversation; it doesn’t finish it.

Most first-time transformation efforts stall within eighteen months — and almost never because the methodology was wrong. It’s because the organization wasn’t ready when the work started. That’s the gap this tool is built to catch, early enough to close it.
